HRT & the Tear Film — Hormone Replacement Therapy and Dry Eye

What the evidence says
The relationship between HRT and dry eye is one of the more nuanced areas in ocular surface medicine. The research does not point clearly in one direction — and we think it's important to tell you that honestly rather than oversimplify.
Oestrogen-only HRT Some studies suggest that oestrogen supplementation may worsen dry eye symptoms in some women — particularly those with existing dry eye disease. A large observational study — the Women's Health Study — found that women using oestrogen-only HRT had a higher prevalence of dry eye symptoms than women not using HRT, with the risk increasing with duration of use.
Combined oestrogen and progesterone HRT The picture with combined HRT is less clear. Some studies suggest a smaller negative effect than oestrogen alone; others show a neutral or modest positive effect on certain tear film parameters.
Androgens Androgen levels decline significantly at menopause and androgen deficiency is associated with meibomian gland dysfunction. Androgen supplementation — not currently a mainstream component of standard HRT — has shown promise in some studies for improving meibomian gland function and dry eye symptoms. This is an active area of research rather than established clinical practice.
What this means practically
HRT decisions are complex and should be made with your GP or menopause specialist based on the full picture of your menopausal symptoms, health history, and risk profile. Dry eye is one factor among many.
What we would say is:
- If your dry eye symptoms started or worsened after beginning HRT, the connection is worth discussing with your prescribing doctor
- If you're considering HRT and have significant dry eye, let your GP know — it's a relevant factor in the discussion
- If you're on HRT and your dry eye is significant, specialist assessment and treatment can produce meaningful improvement regardless of HRT status — the treatments we offer work on the ocular surface disease directly rather than through hormonal mechanisms
The broader hormonal picture
HRT is one piece of a larger hormonal picture that influences dry eye at menopause. For most women, the dominant driver of menopausal dry eye is meibomian gland dysfunction driven by androgen decline — and this responds well to the same specialist treatments that work for other forms of MGD-driven dry eye.
