ZEST stands for Zocular Eyelid System Treatment

It uses a gel formulated from okra — a plant-derived polysaccharide — applied to the eyelid margins with a micro-applicator. The gel works by dissolving and lifting the biofilm, debris, and Demodex-associated material that adheres to the lid margin and lash bases.

It's a professional treatment performed in clinic, taking around 15 to 20 minutes. It's well-tolerated by almost all patients and requires no anaesthetic.

What does ZEST treat?

ZEST is particularly effective for:

  • Demodex blepharitis — removing the collarettes and debris produced by Demodex mite activity, reducing mite populations and the inflammatory response they trigger
  • Anterior blepharitis — clearing the bacterial biofilm and lid margin debris associated with Staphylococcal lid disease

What happens during treatment?

ZEST is straightforward and comfortable.

Before treatment We'll have assessed your eyelid margins thoroughly beforehand — identifying the pattern of disease, the degree of Demodex involvement, and whether ZEST is the right treatment for your presentation.

During treatment You lie comfortably while the okra-based gel is applied to your closed eyelids and worked along the lid margin with a soft micro-applicator. The treatment covers both upper and lower lids. Most patients find it comfortable — some describe a mild tingling sensation. The treatment takes around 15 to 20 minutes.

After treatment The gel is thoroughly rinsed away. Your eyes may feel refreshed immediately. Some patients experience mild temporary redness of the lid margins. You can return to normal activities straight away.

Where indicated, ZEST is followed by meibomian gland expression and/or light-based treatment in the same appointment.

How many sessions will I need?

This depends on the severity of your lid margin disease. Some patients benefit significantly from a single session as part of a broader treatment plan. Others — particularly those with established Demodex infestation or significant biofilm — benefit from a course of sessions spaced a few weeks apart.

After your initial treatment, a structured home maintenance routine is important to maintain results and slow the return of debris and mite populations. We'll build a practical routine with you.

ZEST as part of a broader treatment plan

ZEST works best as part of a comprehensive approach to lid health rather than as a standalone treatment. At our clinic it is typically used alongside:

  • LLLT — blue light therapy to continue reducing Demodex populations and bacterial load between clinic sessions
